- Multiple Sclerosis in Practice: Progress, Challenges, and the Future of the Disease
On the occasion of World Multiple Sclerosis Day on May 30, 2Logical interviewed neurologists from Italy, Portugal, and Spain, with the aim of providing an updated and multidisciplinary overview of the clinical, therapeutic, and organizational evolution in the management of a disease that affects millions of people worldwide. From early diagnosis to artificial intelligence, addressing the challenge of progressive forms, this article provides an integrated view of Multiple Sclerosis (MS), enriched by the contributions of experts such as Dr. Orofino , a medical director of Neurology at the University Polyclinic of Cagliari. More Tools, Greater Complexity All specialists agree: in the last five years, the management of MS has made significant progress. Therapeutic options have increased, and the personalization of approaches has strengthened. “ Management is now heavily oriented towards immunomodulation, much more than it was in the past ,” emphasizes Dr. Orofino , who also highlights the growing use of high-efficacy therapies (HET) in the early stages of the disease. From Portugal, there is an observed acceleration in the initiation of treatments after diagnosis, while Spain shows a commitment towards increasingly patient-centered medicine. At the European level, there is growing attention to progression, brain atrophy, and therapeutic response as new frontiers in clinical monitoring. Progressive Forms and the Need for Biomarkers Despite the successes, neurologists emphasize an urgent need: effective therapies for the progressive forms of MS, particularly Primary Progressive MS (PPMS) . Current therapeutic approaches still show significant limitations in slowing the progression of disability. “We still lack integrated treatments to combat the progression of disability,” a concern that Dr. Orofino shares, also highlighting the lack of a widespread support network, not only in medical terms but also psychological, care, and rehabilitation." New Patient Profiles and Unmet Needs The clinical profile of patients at diagnosis is changing. Neurologists report greater variability in terms of age, symptoms, and clinical form. Some Italian specialists have observed increased immune reactivity, while in Portugal, there is a reported increase in prevalence and greater aggressiveness from the onset. Among the unmet needs are: Treatments for PPMS and SPMS Progression control independent of relapses (PIRA) Better management of invisible symptoms (fatigue, pain, cognitive decline) Access to multidisciplinary teams and rehabilitation programs Personalization of therapies, supported by biomarkers Quality of Life and Psychosocial Impact When asked which aspects of MS are undervalued in research or daily clinical practice, neurologists indicated several areas. The etiopathogenesis of the disease remains a central concern for some, as do environmental factors. Others highlighted “quality of life and patient independence from caregivers,” “cognitive and psychiatric impact,” and “fatigue and changes in the emotional sphere.” The ability to predict relapses was also mentioned. The Ideal Therapy and Vision for the Future In the search for the ideal therapy, neurologists hope for a treatment with "high efficacy, high safety, and practicality (oral or low frequency)." They expect a therapy that also has "high adherence and tolerability," and is capable of "repairing and neuroprotecting." The ability to "prevent progression" is a shared desire. Dr. Orofino believes the ideal therapy should be: effective, easy to use, and well-tolerated; preferably taken at home. The vision for the next 10 years is one of hope but also realism. There is an expectation for "new therapies and new knowledge on etiology," "more patients with MS but with less disability," and "better control of the disease and its progression." The chronicization of the disease with a good quality of life represents a goal, as does the development of “more effective therapies for progressive forms and reparative therapies.” For many Italian doctors, personalized medicine will be the key to the future. The Role of Artificial Intelligence In the context of MS, Artificial Intelligence is seen as a promising tool for the future management of the disease. For Dr. Orofino , artificial intelligence will assist “in the choice of treatments and in predicting long-term effects,” improving the accuracy and timeliness of clinical decisions. Research Priorities If they could guide new lines of research, specialists would focus on: Etiology and environmental factors Therapies for progressive forms and early biomarkers Neuroprotection and remyelination Pathophysiology of MS Dr. Orofino would direct his research towards techniques for delivering drugs to specific sites in the nervous system. Apesar dos progressos, as doenças cardiovasculares continuam a ser das principais causas de mortalidade em Portugal. As doenças do aparelho circulatório foram responsáveis por mais de 32.000 óbitos em Portugal — cerca de 26% de todos os óbitos . As doenças cardiovasculares permanecem como uma das causas de maior mortalidade no país. No estudo RADICAL , quase 9 em cada 10 portugueses possuem pelo menos um fator de risco cardiovascular . 37% da população apresentou 3 ou mais fatores de risco combinados: Prevalências estimadas Na população adulta De acordo com o estudo associado ao INSEF, 37% da população portuguesa entre os 40 e os 69 anos apresenta um risco elevado de desenvolver doença cardiovascular nos próximos 10 anos , e 6% têm risco muito elevado . No total, somando ambos os grupos, 43% das pessoas nesta faixa etária estão em risco elevado ou muito elevado . Apesar das melhorias recentes em alguns indicadores e da evolução favorável da mortalidade por doenças cardiovasculares , Portugal continua a enfrentar disparidades regionais significativas , tanto no controlo dos fatores de risco como na mortalidade entre regiões do país. As doenças cardiovasculares mantêm-se entre as principais causas de morte em Portugal . Os fatores de risco são altamente prevalentes: hipertensão , colesterol , obesidade e sedentarismo . Uma percentagem elevada da população (+40%) está em risco elevado a 10 anos . A discrepância entre regiões e o controlo dos fatores de risco indicam oportunidades claras para intervenções preventivas. Outras ainda já estão num ponto de equilíbrio omnichannel. Medicina Geral e Familiar Diferenças Regionais A Medicina Geral e Familiar (MGF), a especialidade com maior volume , evidencia diferenças regionais marcadas. Lisboa e Vale do Tejo destacam-se pelo peso do digital, o Norte continua quase exclusivamente presencial e o Centro apresenta maior utilização dos canais “Outros”. Norte Predominância absoluta do presencial (acima de 70%). Centro Maior peso em canais Outros, além de equilíbrio relativo. Lisboa e Vale do Tejo Forte abertura digital (~ 50%). Empresas com a Abordagem Mais Equilibrada As empresas que melhor equilibram digital e presencial são também as que apresentam maior diversificação de canais. Líderes de mercado exploram mais de 8 canais distintos , garantindo cobertura em múltiplas especialidades e perfis de HCP. Empresas com maior equilíbrio de canais Estas empresas conseguem distribuir interações de forma balanceada entre F2F e digital. Avaliação Global Estas empresas têm, em média, uma avaliação superior à média do mercado (NPS=42) , confirmando que estratégias omnichannel são não só mais eficazes em alcance, mas também mais valorizadas pelos HCPs. O mercado está em plena transformação omnichannel . O digital cresce, mas o F2F continua essencial. 🎗️🩷 Outubro termina, mas o compromisso com quem vive com Cancro da Mama continua. No âmbito da iniciativa "No cancro, não há perguntas tontas, todas as dúvidas são importantes" , a 2Logical, em colaboração com associações de doentes e com o Grupo Patients Advocacy da Sociedade Portuguesa de Oncologia , desenvolveu um estudo junto de pessoas com Cancro da Mama em Portugal. O objetivo? Perceber que dúvidas os doentes têm, mas nem sempre exprimem, e em que momentos estas surgem com maior frequência. Este estudo procura: Compreender as necessidades de informação ao longo da jornada da doente; Identificar os momentos onde surgem mais dúvidas; Entender por que razão certas questões não são colocadas ao médico; Melhorar a comunicação entre doentes e profissionais de saúde. Cerca de ¼ das pessoas diagnosticadas com Cancro da Mama não sabem o subtipo de cancro que têm/tiveram. 39,1% das pessoas diagnosticadas com Cancro da Mama foram diagnosticadas nos estadios precoces (0 a II). Cerca de metade das pessoas com diagnóstico de Cancro de Mama já receberam o mesmo há mais de 2 anos. Mais de 72% das pessoas diagnosticadas com Cancro da Mama dizem que conseguem expressar sempre ou frequentemente todas as suas dúvidas com o médico, mas há cerca de 17% que dizem que o fazem apenas ocasionalmente ou raramente. A vida sexual, auto-imagem e trabalho são os aspetos mais afetados pelos tratamentos.
